Step 1: Contact Your Insurance Provider

Before you begin, reach out to your insurance company and ask:

• Do you cover out-of-network speech therapy?

• Do you accept Superbills for reimbursement?

• What documents or forms are required?

• How much will I be reimbursed?

• Where do I send the Superbill?

 

Step 2: Get Your Monthly Superbill from Speech Evolution Therapy

We’ll send you a detailed Superbill every month that includes:

• Your name and date of birth

• Provider name, credentials, and NPI number

• Service dates, CPT & ICD-10 codes

• Amount paid and total session cost

Step 3: Fill Out a Claim Form (If Required)

Many insurers ask for a Health Insurance Claim Form alongside the Superbill. You can usually find this on their website—just download, print, and complete it.

 

Step 4: Submit Everything to Your Insurance

Submit your documents by the method your insurance provider accepts:

• Upload via your online insurance portal

• Email or fax the documents

• Mail the Superbill and claim form to their claims address

 

Tip: Keep a copy of everything you send!

Step 5: Monitor Your Claim

Check your insurance portal or call to track your claim. If anything is missing, they’ll let you know. Be ready to provide:

• Additional documentation

• Proof of payment if requested

Step 6: Receive Your Reimbursement

Once approved, your insurance will send you a reimbursement check or apply the amount to your deductible or out-of-network coverage.
